Find education and treatment
Find the programming you need in 3 steps:
- Get your assignment. The court decides what kind of education, treatment, or both you need based on your situation. You get assigned a level. Don’t know which level you need? Contact your probation officer. If you don’t have a probation officer, ask a BHA-licensed treatment provider for help.
- Search for education, treatment, or both. The place you go must be licensed by the BHA. To find an approved provider, use BHA's OwnPath Directory. In the search field, make sure to select "DUI or DWAI education or treatment".
- Enroll. When you find an approved provider from the directory, contact them. Ask about how to enroll in the education or treatment you need.
Get proof you finished (DRS form)
For proof you finished your requirements, you’ll need a DUI/DWAI Referral Summary (DRS form).
To get your DRS form:
- Contact your provider. Reach out to the place where you went for education or treatment. If you need their contact details, search BHA's OwnPath Directory. In the search field, make sure to select "DUI or DWAI education or treatment".
- Request your DRS form. Your service provider should give it to you within 10 business days after you finish at no charge.
You can submit your DRS form to the DMV in one of 3 ways:
- Online at MyDMV Colorado
- Email dor_interlock@state.co.us
- In person at your local DMV
If the place where you went for education or treatment is closed or can’t give you the form, BHA might be able to help. BHA doesn’t accept walk-ins, but you can submit a request to search BHA records. Your request must include a signed consent form to release the confidential record. You can download the consent form in English or español. Then, scan or take a photo of the signed form. Attach your signed form to an email and send it to: cdhs_bha_provider_support@state.co.us.
